Children & Young People’s Social Prescribing Collaborative

The value of leaders at place and neighbourhood level is evident in development of the Children & Young People’s (CYP) Social Prescribing Pilot in South West Shropshire. The Primary Care Network (PCN), in collaboration with the Head of Joint Partnerships & Social Prescribing Lead Advisor, identified an opportunity to create a pilot programme to support their CYP population where there were emerging challenges around social anxiety, school refusals and challenging behaviours. These opportunities were identified through conversations in relation to the existing adult social prescribing offer.
A collaborative of grant funded providers was brought together with the existing social prescribing team and the PCN and grew to include the Early Help & Youth Worker teams in the local area. The impact of the programme on children and young people has been highlighted by community leaders, schools, and parents which in turn has gathered local community and town council support. The opportunities to sustain the programme and develop new creative opportunities have emerged through this support.
