Personalised Care Programmes
Shropshire, Telford and Wrekin

The development of joint partnership roles at system and place levels in Shropshire Telford & Wrekin has created opportunities for whole systems approaches to addressing the ‘wicked issues’ enabling joint decisions to be made based on connections to priorities and health inequalities.

When exploring the benefits of non-clinical approaches to support children and young people with a diagnosis of asthma through Personalised Care links to school readiness, speech & language and early childhood development and supporting perinatal mental health were identified.  The focus on non-clinical creative approaches to meet place and neighbourhood areas of need is held by the Creative Health Programme Manager & Personalised Care Project Manager and the system wide focus is held by the Joint Partnerships Head of Service.
