Live Well Wakefield is a referral based social prescribing and supported self-management service. 

The Live Well Wakefield website holds PDF directories of social and support groups across seven ‘networks’ that people can download and browse.  Directories are not Creative Health specific but include reference to universal and targeted creative activities such as art groups or singing for dementia groups.   Referrals can be sent securely to the Live Well Wakefield team using a web referral form.   A Social Prescribing Assessment Tool that implements NICE guidance recommendations is available for download from the website.
