Social Prescribing Healthy Lives Shropshire

In setting up the social prescribing service in Shropshire, Public Health put in place data sharing agreements with each GP Practice. These agreements allow for patients to be referred by their GP to the Healthy Lives Social Prescribing Service through their digital GP Clinical System EMIS.
From the point of referral, the sharing of information is driven by the patients themselves allowing the social prescribing link workers to update Pharma-Outcomes as well as EMIS.
Data sharing agreements are also in place with providers of non-clinical interventions to allow outcome data to be captured, reported on and digitally shared. These agreements are managed by the community development workers as they update and add providers to the resource bank of social prescribing interventions. Social Prescribing Link Workers will also update Pharma-Outcomes on self-reported improvements.
