Example of: Purposeful data collection to inform delivery and demonstrate impact

The Food Museum Ltd Project
Suffolk and North East Essex

Like many Creative Health projects, The Food Museum Ltd project has developed a ‘mixed methods’ evaluation approach, using both qualitative and quantitative data to capture impact and for this learning to support project development.

The project forms part of the Primary Care Personalised Care Programme and involves a cohort of patients, living in areas with high deprivation requiring physical and mental health support for pre-diabetes, depression, obesity, smoking and higher than recommended alcohol intake.

Each patient is offered an appointment with a Social Prescribing Link Worker, working onsite at the Food Museum where they will develop a ‘what matters to me’ Personalised Care and Support Plan.

Monitoring and evaluation has been developed with Public Health PHM Analysts and includes GAD-7, PHQ-9, and open-ended questions on healthy lifestyles at the final appointment.
