In recognition of the fact that people's health and wellbeing is affected by a range of social issues, the personalised care programme supporting patients with complex needs provides, through social prescribing, a range of non medical preventative interventions including creative health.
This recognises that average annual cost per patient for the top 5% are over 20 times higher than all other patients. The programme has focused to date on people living with two or more long term conditions. Partner organisations are the Food Museum Ltd, Cohere Arts, and Suffolk Art Link.
