Hoxton Creative Health Hub

The Hoxton Creative Health Hub aims to address health inequalities using culture and partnership working. The community collaborates to build both informal and formal referral pathways to culture for those who are not currently accessing it. They benefit from the high number of culture and community organisations clustered in Hoxton and are maximising impact by working collaboratively. 20 organisations are involved in an informal consortium.
They have brought the primary care network, community navigators and social prescribers together with the providers to raise awareness and to work together to increase referrals. Working with the Creative Health Associate, hosted by North East London Integrated Care Board, the meetings have also been attended by Personalised Care teams from the ICB place-based partnership in Hackney. This is an example of how grassroots organisations can come together to influence systems.
Co-leading the Hoxton Creative Health Hub is Hoxton Hall and Hackney Council.
